Meeting the participant at an agreed venue and walk them to a group/activity.
Support individuals to build their confidence, self-esteem and extend their social networks by encouraging a connection
Attend up to 6 sessions with the participant until they are comfortable and confident to access independently.
Develop a good up to date knowledge of local activities, services and opportunities within their local area.
Identify gaps in provision and feed this back to the Community Participation Team.
